U12 Wildhawk 26-27 Tryouts

I hereby consent and agree that TRI COUNTY BASKETBALL INC (SOLDIERS) and Wildhawk Basketball Inc. (WILDHAWK) shall have the unlimited rights to use/publish my child's photograph, video image, voice and likeness in any way they see fit within their productions including publicizing, advertising or trade without restrictions.

SOLDIERS and WILDHAWK are not obliged to compensate any talent for the use of their likeness, unless otherwise agreed upon by SOLDIERS, and WILDHAWK and the talent or talent guardian.

I agree further that any recording, photography, plates or negatives, shall be and remain the exclusive property of SOLDIERS and WILDHAWK and/or its agents and assigns.

I authorize SOLDIERS and WILDHAWK and its employees, agents, and volunteers to provide all medical care which they deem necessary for my child in the event of injury; and I agree to reimburse SOLDIERS and WILDHAWK for all expenses incurred thereby.

In part consideration of permitting my child (listed above) to participate in the activities of SOLDIERS and WILDHAWK I agree to insure my child against any injury or loss resulting from these activities or from traveling to and from these activities. I agree to indemnify and save harmless SOLDIERS and WILDHAWK and its employees, agents, and volunteers from all losses, damages, claims and demands occasioned thereby.SOLDIERS and WILDHAWK are not obliged to compensate any talent for the use of their likeness, unless otherwise agreed upon by SOLDIERS and WILDHAWK and the talent or talent guardian.
