In part consideration of permitting my son / daughter (listed above) to participate in the activities of WILDHAWK BASKETBALL INC. I agree to insure my son / daughter against any injury or loss resulting from these activities or from traveling to and from these activities. I agree to indemnify and save harmless Wildhawk Basketball Inc. and its employees, agents, and volunteers from all losses, damages, claims and demands occasioned thereby.
